Teri S. Formanek, M.D.
Dr. Formanek received his medical degree from University of Iowa. He Completed his Orthopaedic surgery residency at the Creighton-Nebraska Foundation in Omaha, Nebraska. Subsequently, he completed the Harvard Hand Fellowship at Brigham and Women's and Children's Hospital in Boston, MA. He is certified by the American Board of Orthopaedic Surgery with a certificate of added qualifications in hand surgery.

His Orthopaedic Hand Fellowship was completed at the University of Massachusetts in Worchester, MA. Dr. Yanish is Board Eligible in general surgery and has completed his written board certification examination.

Donna J. Bahls, M.D.
Dr. Bahls received her medical degree from the University of Iowa. She completed her Physical Medicine and Rehabilitation residency at the University of Washington in Seattle, WA. She is board certified in Physical Medicine and Rehabilitation and in the American Association of Electromyography and Electro diagnosis.
